Summaries of Benefits and Coverage (SBC)
As part of the Affordable Care Act (ACA), the Federal Government has mandated that group health plans provide plan members a Summary of Benefits and Coverage (“SBC”). These summaries include a notice of modification of plan provisions and a Uniform Glossary of health insurance terms to assist employees with understanding their benefits and coverage options.

The Transparency in Coverage Final Rules issued under the Affordable Care Act require the plan to provide cost-sharing disclosures consisting of in-network provider negotiated rates and historical out-of-network allowed amounts for providers in a machine-readable file format that is available to the public. This data, which is updated monthly, is available at this link: transparency-in-coverage.uhc.com
